The NMC Midwifery Test of Competence is a pivotal assessment required for all midwives trained outside the UK or EU/EEA, and for some UK-trained midwives returning to practice. This evaluation ensures that every individual registering with the Nursing and Midwifery Council (NMC) possesses the knowledge, skills, and values requisite for safe and effective midwifery practice within the UK health system. It is specifically designed for dedicated professionals who want to apply their clinical expertise, meet the highest UK professional standards, and dedicate their careers to supporting mothers and babies across the United Kingdom. This guide is your map for navigating and conquering this essential career milestone.
What the Course Entails and Exam Details
This "course" represents the rigorous process
leading to your registration on the NMC’s register. The entire Test of
Competence is structured around the NMC's Standards for Pre-registration
Midwifery Programmes and the professional Code. The preparation
course or self-study regimen focuses heavily on aligning your existing
knowledge with current UK clinical practice. The curriculum covers fundamental
areas of clinical skills, professional values, critical decision-making,
ethical leadership, and communication strategies in a UK healthcare context.
What to Expect in the Final Exam
The final Test of Competence consists of two distinct but
connected parts, both of which must be passed.
Part 1: The Computer Based Test (CBT)
This is the theoretical arm of the exam, completed at
authorized Pearson VUE testing centers. The CBT is a multiple-choice exam
administered digitally. It consists of 120 questions, which you must complete
within four hours. These questions test both your clinical midwifery knowledge
and, crucially, your ability to apply that knowledge to critical scenarios. The
CBT covers three overarching categories: Professional Values, Communication and
Interpersonal Skills, and Clinical Skills/Midwifery Practice.
Part 2: The Objective Structured Clinical Examination
(OSCE)
The OSCE is the practical assessment, measuring your
clinical competence and interpersonal skills in a controlled, simulated
clinical environment. This part of the exam must be taken in person at an
NMC-approved OSCE test center within the UK. The OSCE involves rotating through
a minimum of ten 'stations' designed to reflect specific clinical scenarios.
Examples of stations include: physiological or complicated birth management, neonatal
resuscitation, postpartum hemorrhage simulation, antenatal assessment, and
professional communication scenarios. You will be evaluated by an examiner on
your ability to apply knowledge, communicate effectively, prioritize safety,
and perform skills to the standard required of a UK-registered midwife.
A passing score is required in both the CBT and the OSCE.
Candidates must pass both components to be eligible to apply to join the NMC
register.
How to Study and Exam Centers
Preparation is paramount, as the exam requires applying
knowledge, not just remembering facts.
Actionable Study Strategies:
Immerse Yourself in NMC
Guidance: Start with the official NMC resources. Read The Code, the Standards
for pre-registration midwifery programmes, and the specific "Midwifery
Test of Competence Handbook." This is your foundational guide.
Use Blueprints: The NMC
provides "test blueprints" outlining exactly which sections of the
standards are covered in the CBT and OSCE. Base your study plan around this
structure.
UK Clinical Practice Review:
Focus on identifying differences between your trained practice and current UK
guidelines, particularly those from the National Institute for Health and Care
Excellence (NICE) and the Royal College of Midwives (RCM). These differences
are often the key to success.
Practice Questions (CBT):
Utilize quality practice question banks that simulate the scenario-based nature
of the CBT questions. Focus on understanding the rationales behind correct and
incorrect answers.
Simulate OSCE Scenarios:
For the practical exam, practice makes perfect. Study the "station
briefs" provided by the NMC. Partner with colleagues to mock-test key
clinical procedures. Focus on speaking your thought process aloud and
communicating with the "patient" or standardized actor exactly as you
would in practice.
