City Carrier Assistant (CCA) Postal Academy Practice Test

The correct answer is that the carrier is responsible for completing the PS Form 3546X if no record exists. This form is used in situations where there is no available record of the item being questioned, such as when a customer inquires about a missing package or service issue. In the postal process, the carrier is typically the first individual to handle the items in question and is therefore in the best position to gather the necessary information directly from the field. The carrier’s familiarity with delivery routes and packages allows them to accurately fill out the form, ensuring that any issues can be addressed promptly. While other entities such as the customer, local post office, or postal supervisor may play roles in the postal system, they are not the ones tasked with completing the PS Form 3546X. Customers may initiate inquiries or request assistance, but they do not have the operational knowledge required to complete postal forms. The local post office and postal supervisors may oversee and manage processes or approve or review forms, but the responsibility for actually filling out the form falls to the carrier who has firsthand knowledge of the delivery in question.
